Abstract:

The United States Bureau of Reclamation (USBR) has been using risk analysis techniques in assessments for management decisions. Before presenting USBR practice and experience, the paper focuses on experienced causes for dam failures. While risk analysis associated with large remote-occurring floods is discussed, actual failures of modern major dams have resulted from causes other than floods, usually 'piping,' and these usually result due to conditions not detected by designers before the failure.
