Abstract:

In many cases of disaster, the causes of failure of huge systems are frequently attributed to human error. However human is liable to error. To exclude human factor from a huge system as little as possible, is a matter of highly importance to assure the safety of large system. The man and system interface should be carefully examined. The paper concerns both sides, man and system. The chain effect of cause and effect relations is effectively explained with the Process Decision Program Chart developed by the author. Safety culture and safety ethics are also emphasized from the point of view of the oriental thoughts.
